Job Description

Position Summary
The CBDS Direct Support Professional (DSP) supports individuals with disabilities in developing vocational, social, and community-based skills that promote independence and employment success. Responsibilities include conducting vocational assessments, facilitating community integration, supporting employment readiness, documenting progress, and providing transportation to community activities and work-related opportunities.
Reports To: Manager – Community Based Day Supports
Essential Responsibilities
  • Support and protect individuals’ rights, dignity, confidentiality, health, and safety.
  • Assist individuals in developing vocational, social, and independent living skills.
  • Conduct assessments to identify interests, abilities, employment goals, and support needs.
  • Facilitate community-based activities, career exploration, volunteer opportunities, and employment readiness programs.
  • Implement Individual Support Plan (ISP) goals and maintain accurate electronic documentation and billing records.
  • Provide transportation and direct support during community and employment-related activities.
  • Collaborate with families, employers, service providers, and community partners to support individual success.
  • Promote inclusive employment opportunities and address barriers to workplace accessibility.
  • Utilize agency technology systems, including documentation, communication, and scheduling platforms.
  • Follow all agency policies, funding source regulations, and Positive Behavioral Supports (PBS) practices.
  • Participate in team meetings, problem-solving, and program development initiatives.
Qualifications
  • High school diploma or GED required; Bachelor’s degree in Human Services, Business, or related field preferred.
  • Minimum one year of experience supporting individuals with disabilities preferred.
  • Strong communication, organizational, and interpersonal skills.
  • Basic computer and documentation skills required.
  • Ability to work independently and as part of a team.
  • Flexible schedule, including evenings or weekends as needed.
  • Valid driver’s license, reliable transportation, and satisfactory driving record required.
  • Successful background check required.
  • Bilingual Portuguese skills are a plus.

Pat Rate: $19-$21 per hourPhysical Requirements
Ability to sit, stand, walk, bend, lift, and assist individuals with mobility needs as required. Must be able to safely transport and support individuals in community settings.
