Licensed Practical Nurse (LPN) – AFC/GAFC Program

Position SummaryThe Licensed Practical Nurse (LPN) provides compassionate, person-centered care to members enrolled in Adult Foster Care (AFC) and Group Adult Foster Care (GAFC) programs. Working under the supervision of a Registered Nurse (RN) or Director of Care Services, the LPN supports clinical care, medication management, documentation, caregiver education, and community outreach efforts while ensuring compliance with all applicable regulations.
Key Responsibilities
1. Professional Conduct & Responsibilities
  • Provide respectful, compassionate, and person-centered care to members, caregivers, and direct care aides.
  • Maintain professional boundaries with members, families, and staff at all times.
  • Demonstrate reliability through punctuality, meeting deadlines, and completing assigned duties.
  • Represent the agency positively through professional appearance, communication, and ethical behavior.
2. Clinical Duties
  • Assist with member assessments, including vital signs, skin checks, medication reviews, and follow-up care.
  • Support the RN in developing, implementing, and updating individualized care plans.
  • Monitor members’ health status and promptly report changes to the RN or Director of Care Services.
  • Provide health education, chronic disease management support, and care coordination to members and caregivers.
  • Develop and track SMART goals to monitor changes in health status and outcomes.
3. Medication Management
  • Adhere to agency policies and state regulations regarding medication administration and documentation.
  • Assist caregivers with safe medication setup, storage, disposal, and refill coordination.
  • Identify, document, and report medication discrepancies or errors promptly.
4. Documentation & Record Keeping
  • Complete accurate and timely documentation, including visit notes, progress notes, MAR reviews, and care plan updates.
  • Utilize the agency’s electronic health record (EHR) system in accordance with policies.
  • Maintain strict confidentiality of member information in compliance with HIPAA regulations.
5. Communication & Collaboration
  • Communicate effectively with members, caregivers, direct care aides, healthcare providers, pharmacies, and interdisciplinary team members.
  • Participate in staff meetings, case conferences, and required trainings.
  • Report critical incidents, safety concerns, and suspected abuse or neglect immediately per agency policy.
  • Support caregiver and direct care aide training through guidance and education.
6. Safety, Compliance & Regulations
  • Follow all MassHealth AFC/GAFC regulations, agency policies, and infection control standards.
  • Identify and report environmental risks or safety concerns within members’ homes.
  • Complete all required trainings (e.g., CPR/BLS, HIPAA, infection control, abuse reporting).
  • Participate in emergency preparedness and safety procedures.
7. Networking & Marketing Responsibilities
  • Participate in assigned weekly or monthly outreach and marketing activities.
  • Represent the agency at community events, senior centers, healthcare facilities, and networking meetings.
  • Build and maintain relationships with referral sources such as social workers, hospitals, and councils on aging.
  • Distribute marketing materials and document outreach efforts and follow-ups in accordance with agency policy.
Qualifications
  • Licensed Practical Nurse (LPN) in good standing (state-specific).
  • Experience in home care, long-term care, or community-based care preferred.
  • Strong communication, organizational, and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Knowledge of AFC/GAFC programs and MassHealth regulations preferred.
Pay Rate: $35 per hour
Work Environment
  • Combination of field-based (home visits, community outreach) and administrative responsibilities.
  • Travel required within service area.
