Independent Living Coordinator
We need a caring Independent Living Coordinator to work with disabled clients. Provide skills training, advocacy, peer counseling, and supportive services. Experience working with troubled individuals is a must. This is a people-centered, team environment. We offer full benefits and the opportunity to work remotely.
Essential Duties Include:
• Provide the core services SCIL offers to individuals with disabilities
• Responsible for intake and assessment. Assist in the development and fulfillment of consumers Independent Living Service Plans and monitor goal achievement.
• Provide information and referral and service coordination between various community providers and state agencies on behalf of individuals with disabilities.
• Provide technical assistance relative to disability laws and consumer rights.
• Provide individual and systems advocacy on issues impacting the disabled community.
• Must have experience with data entry and documentation of notes on services provided using SCIL’s documentation system.
Qualifications:
• Must demonstrate knowledge of disability issues and experience working within human services in a direct relationship with individuals served.
• Must have the knowledge and/or experience working with individuals with a multitude of disabilities
• Must have a valid driver’s license and reasonable transportation means.
• Must have computer knowledge/experience.
• Must possess good verbal and written English skills.
• Must have knowledge of person-centered planning.
• Must have the knowledge and/or experience working with individuals with cross-disabilities and their families.
• Must have knowledge of state agencies and the provider system.
• Must be able to multitask, be self-directed, and have organizational skills to handle tasks.
• Must be fully vaccinated for COVID-19
Education and/or Experience
Bachelor’s degree in social work, human services, psychology, or a related field. This requirement may be substituted for five years’ experience in human services or other community work
